Summer Camp – World Travel

IN STUDIO: I authorize my child(ren) to attend classes at Arts Unleashed at 376 Kingston Rd., Unit 7 in Pickering to participate in hands on Art activities. I understand that with any activity there is the potential risk of accident or injury and do not hold Arts Unleashed, Annika Kim or any Arts Unleashed facilitators liable for any such incident that may occur from the normal use of art materials. I also understand that it is my responsibility to send my child(ren) in appropriate clothing, and even though aprons will be provided, with the use of paint and creative materials there is a chance my children's clothing may be stained or discoloured.

I understand that registration for programs is limited and a 20% deposit is required to secure my space once registration has been completed and an invoice has been received. Monthly payments must be paid in full on or before the start date of each session month. Missed classes will not be credited or refunded. A take home kit will be supplied where possible (some classes are too messy!), or the option to join an alternate class where there is space.
