I authorize my students to participate in an art workshop with Arts Unleashed and participate in hands on Art activities. I understand that with any activity there is the potential risk of accident or injury and do not hold Arts Unleashed, Annika Kim or any volunteers or staff contracted by Arts Unleashed for the workshop, liable for any such incident that may occur from the normal use of art materials at 376 Kingston Rd. Unit 7 or on location at our school. I also understand that it is the school's responsibility to inform parents to send their children in appropriate clothing, and even though aprons will be provided, with the use of paint and creative materials there is a chance students' clothing and / or shoes may be stained or discoloured.
